Key Information on Integrating Medical Care with Community-Based Health and Human Services

Resources outside of traditional clinical care settings can have a significant impact on improving health outcomes. Managed care organizations are actively engaging in partnerships with community-based organizations and centers to meet the needs of Medicare beneficiaries. Medicare Advantage (MA) plans and providers work with organizations to design evidence-based, community-based initiatives offering coordinated care, disease management, and intervention that includes wellness, nutrition, and exercise support to MA beneficiaries.

Better Medicare Alliance will host a briefing on community partnerships in Medicare Advantage that are reducing disease progression and improving health outcomes.
